Forms Not Capturing First and Last Name

From yesterday afternoon all of a sudden the form stopped capturing first and last names.

And so in results those fields are blank. Whilst email and phone are fine.

I did a test and i put names in the form, but again not showing anywhere in the system.

Form is called Bright Debt Advice

Hi there, which account do you have this form on?

Its under the email simon@calvamedia.co.uk

It seems ‘Validate & Submit Step’ is not ticked on your step with the name fields!

Strange as i didnt change anything.

Did you check the box as i see its now got a tick?
