So the dashboard is loading, but when you click the form to edit it, it directs you to this screen? If yes, can you send a message together with the form ID so that we could have a look at it?

I took a look at your form, and everything seems to be working properly on the designer. Could you try accessing the form using an incognito window? If it works there, you probably just need to clear your browser cache.
I also noticed that you’ve added a custom domain for your forms, I’ll be sending a separate email regarding that topic.
