Zapier form completion event not working?

I’m trying to setup a Zapier integration to collect form data into a Zapier table.

However, when I test the step for form completion data, it can’t find anything recent except a nearly empty submission.

How do I this up to get all the fields in the form completion?

we have many users on our zapier integration, this event should be working fine
